Care in the last year of life Alliance Workstream
The Hunter Alliance Care in the Last Year workstream is focussing on three key strategies:
• Shared ICT clinical handover and communication tool
• Community engagement
• Health professional engagement
In January 2015 the Care in the Last Year workstream was been successful in obtaining additional funds from Ministry of Health Planning and Innovation Fund.
The workstream has undertaken a “Let’s Start Talking” campaign (
www.patientinfo.org.au) in partnership with the Local Health Consumer Committees. The campaign includes resources, website and community events and aims to increase community capacity to have conversations about care in the last year of life and increase access to resources.
Let's Start Talking....
The workstream is using patient co-design methodology to inform the development of the shared clinical handover tool – ‘MyNetcare’. Patient, Carers, and staff have been individually interviewed and a solutions workshop was held 19th August.
